29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  At this job site employees were working from a mobile scaffold which did not have a complete platform at a height greater than 10 foot above ground level. Employees were exposed to falls through the scaffold.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  At this job site employees worked from a scaffold platform greater than 10 foot above ground level without fall protection. Employees were exposed to falls to the exterior of the scaffold.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.   At this job site employees climbed the scaffold frames of a mobile scaffold to reach the work platforms and to get down from them. The scaffold frames did not have ladder rungs.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards:   At this job site employees working from a mobile scaffold were not trained by a qualified person to recognize hazardous conditons such as, but not limited to:  A. Improper wear and usage of a personal fall arrest system.  B. Fall hazards.  C. Hazards associated with riding a mobile scaffold as it is pushed.
