On or about July 23, 2013, near 7 feet 9-inch excavation, a ladder for access and egress and access was not available, exposing employees to a 7 feet fall hazard.

On or about July 23, 2013, near the 7 feet 9-inch excavation, the spoil piles were not away from the edge of the excavation by at least two feet, exposing employees to soil rolling back into the excavation.
