On July 24, 2013, an employee was using a Dewalt Circular saw to cut lumber without the protection of safety glasses to protect his eyes from irritants and flying debris. b. On July 24, 2013, employees working on the roof building a new street face for a sign were using a pneumatic nail gun without eye protection, thereby being exposed to a struck by physical object hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On July 24, 2013, employees were using an extension cord that was missing the grounding pin, in order to energize an air compresser that was being used for the pneumatic nail gun. The employees were exposed to contact with live electrical current.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On July 24, 2013, five employees were on the roof of an industrial building without fall protection. The employees were exposed to a fall hazard ranging from 8 to 11 feet.

The employees had previously had fall protection training but did not feel that it was needed for the task. Employees were exposed to a fall hazard.
