29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) West 5th Street and South Beech Street, Mount Carmel, PA; Employees were installing an 8 inch water line in an 8 foot 3 inch excavation that did not provide adequate cave-in protection on two out of the four sides of the excavation, on or about 07/24/2013. Rogele, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 116211103, citation number 1, item number 2 and was affirmed as a final order on 04/19/2010, with respect to a workplace located along Route 34, Newport, PA 17074.
