29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Business Office - On or about 7/19/13, the OSHA 300 log for 2009 contained the following deficiency: 1) An LPN suffered a work related injury on 5/29/09 which resulted in 2 days away from work. The injury was not recorded on the 300 log. b) Business Office - On or about 7/19/13, the OSHA 300 log for 2010 contained the following deficiency: 1) A dietary aid suffered a work related injury on 8/2/10 which resulted in the employee receiving 4 sutures. The injury was not recorded on the 300 log. c) Business Office - On or about 7/19/13, the OSHA 300 log contained the following deficiency: 1) A dietary aid suffered a work related injury on 12/24/11 which resulted in 2 days away from work. The injury was not recorded on the 300 log. d) Business Office - On or about 7/19/13, the OSHA 300 log for 2012 contained the following deficiency: 1) An LPN suffered a work related injury on 1/8/12 which resulted in 1 day away from work. The injury was not recorded on the 300 log. ABATEMENT CERTIFICATION REQUIRED
