29 CFR 1910.26(c)(3)(vii): Ladders should not be used as a brace, skid, guy or gin pole, gangway, or for other uses than that for which they were intended, unless specifically recommended for use by the manufacturer. At the front of this event center employees placed and used a step ladder in a closed position on the platform of a two tier high scaffold for access to the roof. The ladder was not used in the recommended manner by the manufacturer, thus exposing employees to falls from the ladder.

29 CFR 1910.28(d)(7): Guardrails not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1- x 4-inch lumber or equivalent, and toeboards, were not installed at all open sides on all scaffolds more than 10 feet above the ground or floor. Employees used the scaffold platform of a two tier high scaffold to place a step ladder for access to the roof of this establishment. The platform was not equipped with standard guardrails to prevent a fall from the platform; employees were exposed to falls greater than 10 feet above ground level.
