29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):    a. Kenna Elementary School:  Employees were laying and packing block from a scaffold, at heights up to 19 feet above the ground, with wood planks as the platform.  Planks serving as the platform did not extend over the centerline of the support by 6 inches and were not restrained or secured by any means, as observed on 7/23/13.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a. Kenna Elementary School:  Employees were working from a scaffold, to lay and pack block, at heights up to 19 feet above the ground.  The scaffold had not been inspected by a competent person before the shift and several issues needed fixed, as determined on 7/23/13.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).    a. Kenna Elementary School:  Employees were exposed to fall hazards while laying block from a supported scaffold at heights up to 19 feet from the ground without end guarding in place, and missing top or mid-rails, without the use of a personal fall arrest system, as observed on 7/23/13.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:     a. Kenna Elementary School:  Employees were working from a scaffold, laying and packing block, that did not have toeboards erected along the edge of the platforms. There were employees passing back forth underneath, while tools and block were located near the edge of the platform overhead, as determined on 7/23/13.
