29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition): On or about 7/17/2013, Near Maxwell and N. Commerce Streets, an employee who stood in the left lane of south bound N. Commerce did not use a stop/slow paddle, a flag, or an Automated Flagger Assistance Device (AFAD) to stop traffic in the two south bound lanes so trucks could exit the jobsite thereby exposing the employee to struck-by hazards.
