29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   The employer does not ensure that open-sided floors and/or platforms four feet or more above adjacent floor or ground level were guarded with standard railings (or equivalent) and toeboards:.  These violations were observed on or about July 24, 2013, in the Raw Mill Ring area, were employees were exposed to fall hazards during the operation of the Raw Ring Mill where open-sided floors greater than four feet above an adjacent floors were not guarded with standard railings.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrails are installed on all sides of open-sided floors to protect employees from fall hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment that the machine or equipment shall be isolated from the energy source and rendered inoperative.         a)  This violation occurred on or about July 24, 2013, in the 160/160 Ring Mill Area where maintenance employees were exposed to electrical, caught-between and struck-by hazards when performing setup, service and/or maintenance without the machine being isolated from its energy sources.        b) This violation was observed on or about July 24, 2013, in the forging press area where a maintenance employee was exposed to struck-by and caught-in hazards when performing maintenance tasks on a 4000 ton open die forging press without the press being isolated from its energy sources.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that an energy control program is established that consists of energy control procedures, training, and periodic inspections for conducting lockout/tagout activities when performing setup, service and/or maintenance to equipment or machinery.

The employer does not ensure grinding machinery was used with work rest(s) to support offhand grinding work.  This violation occurred on or about July 24, 2013, in the maintenance shop where employees were exposed to struck-by and caught-in hazards when using (2) Bull Bench Grinders to sharpen tool bits without work rests.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rest are installed on grinding machinery used for offhand grinding work.

This violation occurred on or about July 24, 2013, in the maintenance shop where employees were exposed to stuck-by hazards when using two Bull Bench Grinders to sharpen tools bits that did not have a tongue guard installed.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure abrasive wheel machines where the operator standards in front of the machine have tongue guards installed.
