Georgia. The employer did not provide personal protective equipment, such as protective eye wear or a face shield, for the employees cutting stones to size with a 14-inch chop saw.

29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they shall be equipped with such guards when in use.     a) Front yard of 3801 Lake Haven Way, East Point, Georgia. The employer did not ensure that the blade of a 14-inch chop saw, equipped with a diamond tipped blade, was properly guarded. The moveable self - retracting guard was missing from the chop saw.

29 CFR 1926.451(b)(1): 29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, and the employer could not demonstrate that a wider space was necessary:     a) Front of 3801 Lake Haven Way, East Point, Georgia. The upper most working platform of a fabricated frame supported scaffold was not completely planked or decked. Employees were exposed to a fall hazard of 11-feet 10-inches.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:     a) Front of 3801 Lake Haven Way, East Point, Georgia. Employees working from the upper working platform of the fabricated frame scaffold system were not protected from falling approximately 11-feet 10-inches to the ground by the use of guardrails, a safety net system or personal fall arrest systems.

29 CFR 1926.452(c)(2): Tubular welded frame scaffold(s) were not properly braced by crossbracing or diagonal braces, or both, to secure vertical members laterally and to align them so that the erected scaffold was plumb, square, and rigid:     a) Front of 3801 Lake Haven Way, East Point, Georgia. The scaffold frames of the 11-foot 10-inch fabricated frame supported scaffold system were not adequately cross braced to ensure the scaffold frames were plumb, level, and square. The frames had only been braced on the outer frames, the interior of the frame were not braced at all. One of the frames was braced with what appeared to be a synthetic cargo tie down strap.

The legs 11-foot 10-inch high fabricated frame supported scaffolding system were not resting on an adequate foundation. The legs of the scaffold, which were not equipped with base plates, were resting directly on mud sills.
