29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6. Room 122, Respirator Room: On or about 24 July 2013 and times prior thereto, ten DOT-3AL300, scuba air tanks not secured. This condition exposed employees to a struck by hazard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Maintenance bay work area: On or about 24 July 2013 and times prior thereto, the Black and Decker bench grinder, model 9407, tongue guard was out of adjustment. Tongue guard measured 3/8 inch from wheel. This condition exposed employees to a struck by hazard.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm. Maintenance bay: On or about 24 July 2013 and times prior thereto, the 9 foot wide, 12 foot tall, electrically operated roll up door was not equipped with a mechanism to immediately stop the closing process once actuated. This exposed employees to caught-between hazards.
