29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On July 25, 2013, at a jobsite located at 115 SW 3rd St, Lees Summit, MO., employees engaged in tree removal activities were exposed to serious injuries to the legs and torso, in that they were using gas operated chain saws without the use of protective chaps or ballistic clothing. Abatement certification for this violation is not required.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On July 25, 2013, at a jobsite located at 115 SW 3rd St, Lees Sumitt, MO., employees engaged in tree removal activities were exposed to serious eye and face injuries, in that they were using a Tree Chipper Machine without the use of eye and face protection from flying wood particles. Abatement certification for this violation is not required.
