29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch. Maintenance bay work area; on or about 23 July 2013 and times prior; the periphery fan blades on a Sterling Unit Heater, Steam/Hot Water, Model # VS-778, Serial # 898412215005, was not guarded with protective barrier one-half inch or less. The unit is located 16 inches laterally from the stairs and 52 inches from the floor of the mezzanine. The meshing around the blades is approximately 3 by 5 inches. This condition exposed employees to a caught in hazard.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel Maintenance bay work area; on or about 23 July 2013 and times prior; the employer did not ensure that the electrical panels were readily accessible in the event of an electrical emergency. Electrical panel had a broom and buckets stored in front of the panel. This condition exposed employees to an electrical shock hazard.
