29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination:   (a)  Dust Collectors - On or about August 1, 2013, where employees enter permit-required confined spaces containing fall and entrapment hazards created by converging walls and potential atmospheric hazards created by the residual dust, the employer did not  did not create a written certification of the basis of determination for re-classification of the spaces as non-permit-required.   (b)  Raw Feed Hoppers - On or about August 1, 2013, where employees enter permit-required confined spaces containing entrapment hazards created by converging walls and hazardous energy sources, the employer did not create a written certification of the basis of determination for re-classification of the spaces as non-permit-required.   (c)  Blenders - On or about August 1, 2013, where employees enter permit-required confined spaces containing entrapment hazards created by converging walls and hazardous energy sources, the employer did not create a written certification of the basis of determination for re-classification of the spaces as non-permit-required.
