29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. Gibson Cove office; on or about 24 July 2013 and times prior thereto; three instances were noted where the emergency lights were inoperative: Instance (a) - In the storage area; the emergency lights were inoperative. Instance (b) At the bottom of the stairs; the emergency lights were inoperative. Instance (c) In the upstairs area; the emergency lights were inoperative. These conditions exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel. Gibson Cove office; on or about 24 July 2013 and times prior; the employer did not ensure that the electrical panels were readily accessible in the event of an electrical emergency. Two instances were noted: Instance (a) - Circuit breaker panel B, located in the administrative area, has a wall mounted shelf attached covering half of the panel. This condition exposed employees to an electrical shock hazard. Instance (b) - In the storage area; equipment was stored impeding access to the panel. This condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Gibson Cove office, back-up generator room; on or about 24 July 2013 and times prior; the employer did not ensure that the electrical panel was effectively closed. A knockout plug was missing on the left side of the 600 volt electrical panel. This condition exposed employees to an electrical shock and electrical burn hazards.
