29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about July 24, 2013 at the above addressed jobsite, an employee was working below overhead work was not wearing a hard hat to protect against falling material and debris. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about July 24, 2013 at the above addressed jobsite, an employee was working on an elevated platform with no guardrails or fall protection. The employee was exposed to a fall hazard of approximately over 25 feet from ground. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck (C) Protection from falling objects as indicated necessary by the operating conditions. On or about July 24, 2013 at the above addressed jobsite, an employee was working off a pallet. The platform was not secured to the lift, the employee had no controls of turning the truck off, and protections for falling obects was not provided. The employee was exposed to a fall hazard over 25 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
