29 CFR 1910.107(b)(5)(i): Visible gauges or audible alarm or pressure activated devices were not installed to indicate or insure that the required air velocity is maintained:     a) Spray booth, Stiles Enterprises, Inc., 114 Beach Street, Suite 1W, Rockaway, NJ 07866.    Workers spray adhesive that contains flammable liquids to include, but not limited to hexane, toluene, acetone, and methyl ethyl ketone.  The spray booth was not provided with a manometer to show the pressure drop across the filter and to indicate that the booth was functioning properly.      Violation was observed on or about 7/26/2013.

The spray booth was not provided with an automatic extinguishing system on the downstream side of the filters for wokers spraying adhesive that contains flammable liquids to include, but not limited to hexane, toluene, acetone, and methyl ethyl ketone.           Violation was observed on or about 7/26/2013.

29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded on fourth inch:    a) Work area, Stiles Enterprises, Inc., 114 Beach Street, Suite 1W, Rockaway, NJ 07866.    Employees were exposed to being struck by wheel fragments when using a Jet Electric grinder without a tongue guard to sharpen blades used for cutting belts.      Violation was observed on or about 7/26/2013.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Work area, Stiles Enterprises, Inc., 114 Beach Street, Suite 1W, Rockaway, NJ 07866.    The employer failed to ensure that employees who voluntarily wear N-95/Organic Vapor filtering face-piece respirators  were provided with Appendix D information to include but not limited to use, care and limitations of respirators.  Workers use such respirators during adhesive spraying operations.    Violation observed on or about 7/26/2013.
