29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On or about July 23, 2013, and at times prior there to, an employee was observed working from a man-made scaffold that had a walking surface that was not at least 18 inches wide. The employees were exposed to a fall of greater than 6 feet to the ground below.
