29 CFR 1926.106(d): A lifesaving skiff was not immediately available at locations where employees were working over or adjacent to water:       a)    At the worksite, Interstate 24 Bridge over the Ohio River, employees were working on the underneath side of the bridge, out over the river.  They had previously installed the wooden work platforms and now were working off the platforms.  They used Personal Fall Arrest Systems to build the platforms.  A skiff was not onsite to facilitate any rescue operations while they were working over the river water.
