29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: a. Worksite - On 07/23/2013 the platform extended beyond the supports by approximately 60 inches at one end. This cantilevered portion of the platform was used to support the end of another scaffold platform perpendicular to this one. b. Worksite - On 07/23/2013 the platform extended beyond the supports by approximately 30 inches at the other end.

The pole was placed on two stacked bricks on top of a single 4 x 6 supported by other lumber. The support was placed on uneven soil and was not level. b. Worksite, front of building - On 07/24/2013 the support under a second pole of a pump jack scaffold was not level, sound, or rigid. The support was placed on uneven soil, and was not in full contact with the soil.
