29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the shop area, machinery was not properly guarded, exposing employees to amputation hazards in the following three instances: 1) Hitachi Seiki (Model 4NE-600) CNC Lathe had the safety interlock switch wired in the closed position, which prevented the machine from being disabled when the door is opened during operation. 2) IKEGAI (Model FX25N) CNC Lathe had the safety interlock switch wired in the closed position, which prevented the machine from being disabled when the door is opened during operation. 3) Kalamazoo Band Saw was missing a guard over the blade, with 8 1/2 inches of exposed blade directly right of the cutting area.
