29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  In the shop area, specific lockout/tagout procedures had not been developed,, exposing employees to the hazards of 208 VAC/3-phase power and pneumatic air pressure, creating an electrical shock and struck by hazard in the following two instances:  1) Hitachi SEIKI CNC Lathe, Model 4NE-600.  2) IKEGAI CNC Lathe, Model FX25N.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  In the shop area, machinery was not properly guarded, exposing employees to amputation hazards in the following three instances:  1) Hitachi Seiki (Model 4NE-600) CNC Lathe had the safety interlock switch wired in the closed position, which prevented the machine from being disabled when the door is opened during operation.  2) IKEGAI (Model FX25N) CNC Lathe had the safety interlock switch wired in the closed position, which prevented the machine from being disabled when the door is opened during operation.  3) Kalamazoo Band Saw was missing a guard over the blade, with 8 1/2 inches of exposed blade directly right of the cutting area.

29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  In the shop area, safety guards were not covering spindle ends, wheels and flange projections, exposing employee to a struck by and caught in hazard in the following two instances:  1)  The Rockford Bench Grinder had not side guards over the left and right wheel.  2)  The Dayton Tool Grinder had no guard over the grinding wheel, spindle ends or the flange projections.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  In the shop area, the Rockford bench grinder was missing work rests on the left wheel and the right wheel was adjusted to approximately 1/2 inch gap from the wheel to the work rest, exposing employees to a struck by hazard.
