29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a)Third Floor: Laundry/Vending Area - A carpenter was exposed to a shock/electrocution hazard as energized power circuits [480 volts] were not de-energized and grounded or effectively guararded by insulation or other means. An employee was installing sheet rock/wallboard within several inches of an energized Alumna Flex/SEC cable that contained abraded sheathing. The employer did not adequately evaluate the installation and condition of the service entrance cable. This was observed on June 28, 2013.

Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. a) Third Floor Vending/Laundry Room - The employer instructed their employee to install sheet rock/wallboard at the construction site. An employee worked adjacent to or within several inches of an energized conductor. The service entrance cable [SEC] was not properly installed and protected in accordance with the National Electrical Code,Article 338.10 (B)(4)(a). This was observed on June 28, 2013.
