This violation was observed on or about July 24, 2013 and at times prior thereto, at the facility where an employee serving as attendant was not trained to know the hazards that could be faced during entry.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that affected employees are provided information and training which would allow them to identify the modes, signs or symptoms, and consequences of an exposure while serving as the attendant during a confined space entry.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     The employer does not provide adequate eyewash facilities for employees exposed to injurious, corrosive chemicals in the workplace.   This violation was observed on or about July 25, 2013, on the Purge Rack where employees were exposed to eye/skin contact with hazardous chemicals such as anhydrous ammonia while extracting the product from a rail car.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that eyewash stations are properly functioning so that they can provide quick drenching or flushing facilities during an emergency situation.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees exposed to hazardous chemicals during the course of their work are provided information and training on these chemicals before initial assignment to the work area.
