29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.     On or about June 5, 2013, employees were exposed to fall hazards in that the employer did not ensure that employees who were installing shingles on a 2 and 12 pitch roof above 6 feet were protected from falling by a guardrail system, safety net system, or a personal fall arrest system.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about June 26, 2013, employees were exposed to fall hazards in that the employer failed to insure that employees who were assigned the task were instructed in the requirements of fall protection, including, but not limited to a guardrail system, safety net system, or a personal fall arrest system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about June 5, 2013, employees were exposed to fall hazards in that the employer did not ensure that the ladder side rails extended atleast 3 feet above the upper landing.
