29 CFR 1926.1053(b) Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed.   a) Front Lawn, Center of Structure - Worker in an 11' deep trench, used a stepladder to exit and enter  that was 6' short of reaching the outer landing of the trench, on or about 7/23/13.
