29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: a) LA Fitness Job Site - On or about 7-29-13 - Employees were working from fabricated metal scaffold system that was not properly supported. One leg of the scaffold system was set on a cracked plank set over a hole that was filled with a solid concrete block and piece of form insulation under the plank.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) LA Fitness Job Site - On or about 7-29-13 - Employees were exposed to 12 to 13 foot fall hazard from scaffold platform while laying brick, no access ladder was being used. Employees were climbing the scaffold frame and cross bracing to access platforms.
