29 CFR 1910.1030(d)(3)(viii): When personal protective equipment was removed, it was not placed in an appropriately designated area or container for storage, washing, decontamination or disposal, after removal:   At the DiversiCare of Sedgwick care facility at 712 N Monroe Avenue in Sedgwick, Kansas respiratory protection used in the laundry area was not maintained in a sanitary and reliable condition in that disposable masks were stored without the benefit of being sealed in an enclosure:    (a)   A protective mask was stored on top of the laundry detergent supply manifold in the laundry room.   (b)   A protective mask in the laundry room was stored by hanging it on a hook by its elastic retention strap.  This diminished the elastomer qualities and effectiveness for the next use.
