29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying hazards associated with aerial lifts).      (a)   Greater Western Fence, LLC, 58th St. & Sheridan Blvd., Arvada, CO 80002: On and before July 26, 2013, the employer did not ensure that a competent person was onsite and evaluating the hazards associated with fencing operations. 29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:      (a)    Greater Western Fence, LLC, 58th St. & Sheridan Blvd., Arvada, CO 80002: On or before July 26, 2013, employees were seen using two ladders improperly. The employee used two stand alone ladders and placed one foot on one ladder and the other foot on the other ladder. This condition exposes the employee to fall hazards.
