For employees working at 4524 Tice Street in Fort Myers (FL) during demolition activities required to use half face tight-fitting respirators with P100 cartridges, as observed on or about 07/24/2013. The employer had not established and implemented a written respirator program that included elements such as, but not limited to: medical evaluations, fit-testing and training.
