OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards associated with improper automobile lift operations:    a) On or about July 26, 2013 and at times prior to that date, employees were exposed to crushing and struck-by hazards while working around automotive lifts as follows:        - Lifts were not inspected periodically by a qualified inspector.       - Lifts were not inspected daily during use to ensure that they were properly maintained and functional.      - Lifts were not locked and supported with vehicle stands while servicing vehicles.      - Employees not using adapter pads while lifting vehicles on the lifts.      - Employees were not properly trained on operating the vehicle lifts.      - Lifts were not periodically maintained in accordance with the recommendation of the lift manufacturer.       - Lifts were not provided with warning labels indicating their load limit and rated capacity.     Abatement note:   Feasible and acceptable methods to correct these hazardous conditions would be to follow the requirements in accordance to the instructions required in the ANSI/ALI ALOIM: 2008 Safety requirements for operation, inspection, and maintenance standard which includes but is not limited to the following:       1. Institute a periodic preventive maintenance program.        2. Lifts must be inspected by a qualified inspector.         3. Conduct and document periodic inspections and maintenance.        4. Retrain the employees' on the proper operating procedure for the vehicle lifts.        5. Provide the lifts with identification plates indicating their Model and Serial number.       6. Provide warning labels indicating their load limits and rated capacity.

29 CFR 1910.253(b)(4)(i):     Oxygen cylinders were stored near highly combustible material, especially oil and grease:        a) On or about July 26, 2013 and at times prior, employees were exposed to explosion injuries from an oxygen cylinder stored together to an acetylene cylinder inside a compressor/oil room.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.   a) On or about July 26, 2013 and at times prior, employees were exposed to fire hazards from several soiled rags that were left in the service bay area. The soiled rags were not put inside the designated red metal container can.
