29 CFR 1926.453(b)(2)(iv):  Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:                                                                                                                                               a)  Jobsite, 1049 Freeport Road, Creighton, PA 15030:  Observed on or about July 29, 2013, an employee working in the basket of a JLG articulating boom aerial lift (model number 450AJ SII, serial number 0300067693) was standing on the midrail of the basket while installing metal roofing material.  The employee was exposed to a fall hazard of approximately 25 feet to the paved surface below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:       a)  Jobsite, 1049 Freeport Road, Creighton, PA 15030:  Observed on or about July 29, 2013, an employee working in the basket of a JLG articulating boom aerial lift (model number 450AJ SII , serial number 0300067693) was not wearing a body belt with a lanyard attached to the boom or basket.  The employee was exposed to a fall hazard of approximately 25 feet to the paved surface below while working from the basket of the lift.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:       a)  Jobsite, 1049 Freeport Road, Creighton, PA 15030:  Observed on or about July 29, 2013, an employee engaged in roofing activities on the roof of a commercial building was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.  The slope of the roof ranged from approximately 5 in 10 to 10 in 12 (vertical to horizontal).  The employee was exposed to a fall hazard of approximately 25 feet to the paved surface below.
