29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a) PAM Area, 900 Hemlock Road, Morgantown, PA- Employees were performing work activity and were not protected from stepping into a hole/pit. The hole/pit had a skid partially covering it and was configured in an L shape that measured approximately 109 inches long by 36 inches wide and 24 inches deep, thereby exposing employees to injuries, on or about July 15, 2013. No abatement certification or documentation required.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: (a) PAM (Plasma Ark Melt) Area, 900 Hemlock Road, Morgantown, PA - A large open-sided pit/floor area was not adequately protected with a guardrail system in that a mid-rail was missing, thereby exposing employees to a fall hazard of approximately 35 feet to the bottom of the pit, on or about July 15, 2013. No abatement certification or documentation required.
