29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On July 26, 2013 and at times prior thereto; front of site, employees were operating a table saw which was not equipped with an anti-restart/magnetic switch to prevent it from accidental start up if the power was lost and re-engaged.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   a) On July 26, 2013 and at times prior thereto; front of building, employees were observed on the working on the platform that was not fully planked, missing several planks; the height from the work platform to ground level was 22 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) On July 26, 2013 and at times prior thereto; front area, the ladder used to access the scaffold was not extended three feet above the platform exposing the employees to a fall hazard.
