29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation. On or about July 30, 2013, The Franciscan Sisters of Chicago Service Corporation dba The Village at Victory Lakes, Lindenhurst, IL, did not adequately complete the OSHA Form 300 Log of Work-Related Injuries and Illnesses as required for calendar year 2013. a) On or about April 9, 2013, a recordable injury (Entry #3 on 2013 OSHA 300 Log) occurred when an employee incurred a back injury. The 2013 OSHA 300 Log did not adequately describe the injury, or the object/substance that directly injured the person. b) On or about May 20, 2013, a recordable injury (Entry #5 on 2013 OSHA 300 Log) occurred when an employee incurred an injury to fingers. The 2013 OSHA 300 Log did not adequately describe the injury nor specify which hand (ie. the right hand) or which fingers (ie. the right index and right middle fingers) were iaffected. c) On or about June 4, 2013, a recordable injury (Entry #6 on 2013 OSHA 300 Log) occurred when an employee incurred an injury. The 2013 OSHA 300 Log did not describe the injury or specify the body part affected. d) On or about June 18, 2013, a recordable injury (Entry #7 on 2013 OSHA 300 Log) occurred when an employee incurred a shoulder strain. The 2013 OSHA 300 Log did not specify which shoulder (right or left) was affected. e) On or about June 27, 2013, a recordable injury (Entry #9 on 2013 OSHA 300 Log) occurred when an employee incurred an elbow strain. The 2013 OSHA 300 Log did not specify which elbow (right or left) was affected. f) On or about July 2, 2013, a recordable injury (Entry #11 on 2013 OSHA 300 Log) occurred when an employee incurred an infection from a scratch. The 2013 OSHA 300 Log did not specify the body part affected. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
