The 2013 OSHA 300 Log did not adequately describe the injury nor specify which hand (ie. the right hand) or which fingers (ie. the right index and right middle fingers) were iaffected.   c) On or about June 4, 2013, a recordable injury (Entry #6 on 2013 OSHA 300 Log) occurred when an employee incurred an injury. The 2013 OSHA 300 Log did not describe the injury or specify the body part affected.   d) On or about June 18, 2013, a recordable injury (Entry #7 on 2013 OSHA 300 Log) occurred when an employee incurred a shoulder strain. The 2013 OSHA 300 Log did not specify which shoulder (right or left) was affected.   e) On or about June 27, 2013, a recordable injury (Entry #9 on 2013 OSHA 300 Log) occurred when an employee incurred an elbow strain. The 2013 OSHA 300 Log did not specify which elbow (right or left) was affected.   f) On or about July 2, 2013, a recordable injury (Entry #11 on 2013 OSHA 300 Log) occurred when an employee incurred an infection from a scratch.
