29 CFR 1926.452(c)(2): All brace connections were not secured: (a) On or about July 29, 2013 - 502 North Main Street, Arab, AL, an employee was working from fabricated frame scaffolding that did not have all cross braces connected causing the scaffold to be uneven and leaving an aluminum work platform unable to fit securely.

29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches: (a) On or about July 29, 2013 - 502 Main Street, Arab, AL, an employee was working from fabricated frame scaffolding where the aluminum work platform was not securely attached to the scaffold nor did it extend over the end of the frame by 6 inches or more.
