29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. In the staff lounge area, the General Electric refrigerator was not grounded. The duplex outlet the refrigerator is plugged into was not grounded. The hot and neutral wires on the outlet may be crossed. 2. In the staff lounge area, the General Electric toaster oven was not grounded. The circuit that the toaster oven is plugged into was not grounded. The hot and neutral wires on the outlet may be crossed. 3. In the staff lounge area, the General Electric microwave oven was not grounded. The circuit that the microwave oven is plugged into was not grounded. The hot and neutral wires on the outlet may be crossed.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:(a)(LOCATION)(IDENTIFY EQUIPMENT AND CONDITION(S))(STATE HAZARD(S) TO EMPLOYEE(S)) 1. Inside of the compressor room, there was white romex wiring being used to energize the compressor. There was a switch box connected to the romex and the romex was plugged into a duplex outlet. The romex was connected to the compressor and extended to the duplex outlet. The compressor wire needs to be hard wired inside of conduit.

29 CFR 1910.1030(d)(2)(viii): Immediately or as soon as possible after use, contaminated reusable sharps shall be placed in appropriate containers until properly reprocessed. 1. Employees leave the operatories (procedure rooms) with needles and have to walk these needles to the x-ray room. The sharps containers are located under the counter in the film developing closet. All contaminated sharps have to be walked to that area from the operatories, increasing chance of injury to employees.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: In completed installations each outlet box shall have a cover, faceplate, or fixture canopy. 1. Inside of the employee lounge, on the wall near the General Electric refrigerator, there was a cover missing on the light switch. 2. Inside of the employee restroom, there was a cover missing from the junction box located on the ceiling. 3. Inside of the compressor room, there was a cover missing from the electrical box located on top of the compressor.
