29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    Establishment Wide: On August 8, 2013, and at times prior thereto, the employer did not ensure that all employees who were included in the Hearing Conservation Program and worked in areas where they were over-exposed to an eight hour time weighted average of 85 dBA or above of noise were provided with annual audiograms.

29 CFR 1910.134(d)(1)(ii): The employer did not use a NIOSH-certified respirator in compliance with the conditions of its certification:   Establishment Wide: On July 30, 2013, and at times prior thereto, the employer did not prevent employees from modifying their supplied air respirators by installing an air-flow control valve, not approved by the manufacturer, thereby negating the respirator certification.   This standard is incorporated into shipyard employment by 29 CFR 1915.154.

29 CFR 1910.134(i)(7): Oil lubricated compressor(s) used to supply breathing air did not have a high-temperature or carbon monoxide alarm(s) or both:    Establishment Wide: On July 30, 2013, and at times prior thereto, the employer did not ensure that an oil lubricated compressor was equipped with either a high temperature or carbon monoxide alarm. In addition, no carbon monoxide monitoring of the breathing air was being conducted.   This standard is incorporated into shipyard employment by 29 CFR 1915.154.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting face piece respirator were not annually fit tested:   Establishment Wide: On July 30, 2013, and at times prior thereto, the employer did not ensure that employees who were required to wear full face supplied air Abrasi-Blast respirators were provided with annual fit tests.   This standard is incorporated into shipyard employment by 29 CFR 1915.154.  Delta Sandblasting Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(1), which was contained in OSHA inspection number 313586851, citation number 01, item number 003 and was affirmed as a final order on 12/02/2009, with respect to a workplace located at Coast Guard Island, Cutter Sherman, Alameda, CA 94501.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed:  Establishment Wide: On July 30, 2013, and at times prior thereto, the employer did not ensure that the tag indicating the date of the maintenance change and signature of the authorized person had been placed on the Clemco Breather Pot.   This standard is incorporated into shipyard employment by 29 CFR 1915.154.
