29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:   (a)  BB Roofing LLC at 1027 Kalamath St, Denver, CO 80204:  On July 12, 2013 the exposing employer did not ensure that employees were protected from injury, in that one employee installing roofing material and shingles on the second level roof of a two-story residential home was wearing a fall harness; however, the harness was not attached to the lanyard of the fall protection system. The pitch of the roof was 5-in-12. This condition exposed the employee to a fall hazard of approximately 20 feet.    (b)  BB Roofing LLC at 1027 Kalamath St, Denver, CO 80204:  On July 12, 2013 the exposing employer did not ensure that employees were protected from injury, in that one employee cleaning debris from the gutter on the second level roof of a two-story residential home was wearing a fall harness; however, the harness was not attached to the lanyard of the fall protection system. The pitch of the roof was 5-in-12. This condition exposed the employee to a fall hazard of approximately 20 feet.
