29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) In the Offset Area (Main Building), a forklift was found to be in need of repair to the seat and seatbelt. Employees were exposed to possible personal injury on or about August 1, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): (a) At the Bayard Printing Group locations (301 W. Main St. Plymouth, PA 18651 and 180 W. Main St. Plymouth, PA 18651): There were numerous chemicals throughout the facility that contained ingredients (such as VOCs), which may cause adverse health reactions. Employees were exposed to various highly toxic chemicals on or about August 1, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
