A clip-on chuck, in-line valve with pressure gauge and sufficient length hose was not provided and/or used to allow an employee to stand outside of the trajectory while inflating a tire(s) of the Caterpillar forklift (model P6000; s/n AT13F03302).

29 CFR 1910.177(f): The employer did not establish a safe operating procedure for servicing multi-piece rim wheels which contained, at a minimum, the elements listed in 29 CFR 1910.177(f)(1) through (11): a) Lakeside Cabins, Ltd. - Shiloh, Ohio: On or about July 17, 2013, the employer failed to establish a safe operating procedure which would protect employees from struck-by hazards when servicing multi-piece rim wheels. Safe procedures to deflate and inflate tires and to handle multi-piece rim wheels outside of the trajectory were not implemented. An employee sustained serious injuries when servicing rim wheels of the Caterpillar forklift (model P6000; s/n AT13F03302).
