29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Jobsite, 1300 Monastery Drive, Latrobe, PA:  On or about 07/31/13 employees engaged in residential construction activities approximately 9 feet above the ground were not protected from falling by guardrails, safety nets or personal fall arrest systems.

29 CFR 1926.503(c)(3): The employer did not retrain affected employees when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:   (a) Jobsite, 1300 Monastery Drive, Latrobe, PA:  On or about 07/31/13 the employer did not retrain employees who were working without fall protection.  The failure to use fall protection indicated the employees had not retained the requisite understanding and skill to protect them when exposed to fall hazards.
