29 CFR 1910.25(d)(1)(i): Ladders shall be maintained in good condition at all times, the joint between the steps and side rails shall be tight, all hardware and fittings securely attached, and the movable parts shall operate freely without binding or undue play: On or about July 31, 2013, in the facility, employees (Material Handlers) required to manually fill the feed hoppers on the plastic mold machines were exposed to a fall hazard due to the use of unstable (wobbly) portable wood shop-made stepladders (3-4 steps) when accessing the hoppers.

29 CFR 1910.25(d)(1)(x): Ladders shall be inspected frequently and those which have developed defects shall be withdrawn from service for repair or destruction and tagged or marked as "Dangerous, Do Not Use." On or about July 31, 2013, in the facility, employees (Material Handlers) required to fill the feed hoppers on the plastic mold machines were exposed to a fall hazard due to the use of unstable (wobbly) portable wood shop-made stepladders (3-4 steps) when accessing the hoppers. Ladders were not inspected frequently and withdrawn from service for repair or destruction.

29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: On or about July 31, 2013, in the facility, authorized employees were required to operate propane-powered forklifts during production operations. The employer did not certify that each operator had been evaluated as required by paragraph (l).
