29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a)    Dean's Body Shop failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 for hazards associated with; 2) a written respiratory program including physical evaluations, fit testing and employee training for employees required to wear 3M tight fitting face piece respirators with organic vapor cartridges and for Citation 2 for hazards associated with; 1) employees wearing 3M 8210 dust mask for voluntary usage who were not provided information in Appendix D of 1910.134; and 3) a written hazard communication program which included labeling requirements for all containers of hazardous chemicals and employee training on chemicals such as but not limited to hexamethylene diisocynante.

29 CFR 1910.107(c)(5):  Electrical equipment not approved for locations containing both deposits of readily ignitable residues and explosive vapors was present in spraying area(s):    (a)  The boom box stereo system in the paint room was not approved for locations where paint was sprayed.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.107(c)(6): Electrical wiring and equipment not subject to deposits of combustible residues but located in a spraying area as herein defined were not explosion-proof type approved for Class I, group D locations and did not otherwise conform to the provisions of subpart S of this part, for Class I, Division 1, Hazardous Locations:    (a)  The lights on the walls and ceiling of the paint spray area were not appropriately enclosed to prevent combustible residues from contacting exposed wires.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a)  The employer did not provide a written respiratory protection program which included physical evaluations, fit testing and employee training for employees who were required to wear 3M tight fitting face piece respirators with organic vapor cartridges when painting car bumpers and fenders in the paint spray area of the body shop with paint that contained isocyanates.
