The employer did not provide Hazard Communication training to employees using Dryvit, sand pebble pastel base. 29 CFR 1910.1200 paragraph (h). b. The employer did not develop or maintain a log containing Material Safety Data Sheets (MSDS) for chemicals used at the site. Chemicals used include, Dryvit pastel base. 29 CFR 1910.1200 paragraph (g).

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards: On and before 07/25/2013, training was not provided to employees operating powered industrial equipment at the site. Equipment includes the following: a. The Genie: articulating aerial lift, model # S-60. b. Scissor lift: Model GS-3246.
