29 CFR 1926.451(a)(2): Direct connections to roofs and floors, and counterweights used to balance adjustable suspension scaffolds, were not capable of resisting at least 4 times the tipping moment imposed by the scaffold operating at the rated load of the hoist, or 1.5 (minimum) times the tipping moment imposed by the scaffold operating at the stall load of the hoist, whichever is greater:     a) A two-point suspension scaffold was provided for employee use with counterweight materials did not achieve a 4:1 safety factor.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels by personal fall arrest systems, covers or guardrail systems erected around such holes:       a) Employees were exposed to the hazard of falling approximately 16 feet through unprotected skylights located on the roof deck.      No abatement certification or documentation required for this violation.
