29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On the northeast side of the building, employees installing a metal roofing system on an unprotected steep roof were not protected from falling as no fall protection measures were in place to protect the employees from a fall hazard of approximately 13-feet, 6-inches to the next lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On the northeast side of the building, employees installing a metal roofing system on an steep slope roof were using a extension ladder to access the roof. The ladder side rails did not extend at least 3 feet above the upper landing surface, exposing the employees to a fall of approximately 13-feet, 6-inches to the next lower level.
