29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) Axle department; The drill press machine (model #79312888851) was not equipped with a guard, exposing employees to amputation hazards. The employees used the machine to construct starter holes within the metal. (b) Maintenance department; The drill press machine (model #J-A5818) was not equipped with a guard, exposing employees to amputation hazards. The employees used the machine to fabricate holes within the metal parts. (c) Fabrication department; The employer exposed employees to amputation hazards while using a powered press brake (Niagara HBM100810) which was not equipped with point of operation guarding. (d) Fabrication department; The employer exposed employees to amputation hazards while using a powered press brake (Cincinnati 110M) which was not equipped with point of operation guarding. (e) Door department; The employer exposed employees to amputation hazards while using the seaming machine that was not equipped with point of operation guarding. The machine was used to seam two components of metal together.
