29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments:       On or about July 28, 2013 and at times prior thereto, employees performing water well pump removal were not adequately trained to take the steps necessary to deenergize all electrical sources and ensure the electrical sources remained deenergized while setting up the Pul-A-Pump machine exposing employees to the hazards of contact with energized electrical equipment.

29 CFR 1910.333(b)(2)(i):  The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives.       On or about July 28, 2013 and at times prior thereto, Pump and Supply LLC did not create written specific procedures for the safe steps for the deenergization and verification of deenergization of electrical energy sources prior to the process of setting up the Pul-A-Pump machine exposing employees to the hazards of contact with energized electrical equipment.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag was not placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section.      On or about July 28, 2013 and at times prior thereto, employees did not affix lockout devices to the circuit breaker hat controls the water well pump exposing employees to the hazards of unexpected energization of electrical equipment.

29 CFR 1910.333(b)(2)(iv)(B): A qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized.     On or about July 28, 2013 and at times prior thereto, employees performing deenergization tasks such as, but not limited to, turning off circuit breakers were not required to verify the deenergization of electrical energy sources exposing employees to the hazards of electrical shock.
