29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) Engine Room: The eyewash/shower unit located in the #1 engine room was not accessible for employees usage, in that it was obstructed by a chair and other articles exposing workers to the hazards of ammonia. In accordance with ANSI/IIAR Standard 2.2008, Section 13.3.1.4; IIAR Bulletin 109, Section 4.10.10. 1997 Edition; and OSHA 29 CFR 1910.151 (c).

29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices (note: this standard applies to pressure vessels and storage tanks; piping systems (including piping components and valves); relief and vent systems and devices; emergency shutdown systems; controls (including monitoring devices and sensors, alarms, and interlocks) and; pumps as referenced by sections (j)(1)-(j)(1)(vi) of this standard. a) Engine Room #1: The employer failed to ensure that the ammonia sensor was calibrated per manufacturer guidelines. Employees were exposed to the hazards associated with ammonia, in that the ammonia detector did not receive its annual calibration that was due on May 20, 2013.

Employees were exposed to the hazards of ammonia while performing patching and repairing leaks, clean-up operations from spills and other situations involving small emergency operations.
