29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) First Floor: Four fans were not grounded due to missing ground pins.  b) Second floor: There were two floor fans were not grounded due to missing ground pins.    c) Break Room:  The snack machine was not grounded due to a missing ground pin.  d) First Floor: A batteriy charger was not grounded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  First and Second Floor: Strain reliefs failed on floor fans. Cord jackets separated from plugs exposing insulated conductors.
